Vermiculite method

I have been having decent success germinating seeds in plastic containers with vermiculite, however some have tended to go moldy or grow the dreaded fur!

I figured the medium is possibly too damp.

Do people leave the lids of their containers to get some air in?
How much vermiculite do people put in the containers?
Do you place the seeds on top of put them under the medium?
I soak mine for a few days in water, do you bleach them too?

The worst culprits for mould at Trachycarpus. In the two day soaking should this water contain the 10% bleach solution or should I do this only for a few minutes?

Any other tips on this germination method would be appreciated. I want to make sure I am doing it correctly.
